The Hillsborough side relied on a shootout to seal their win and will be joined by Queen’s University, Banbridge and Instonians in the final four.
It was Cookstown who took the lead in the tie with seven minutes played when Michael Kerr latched on to a rebound from Daniel Kerr’s initial strike before slapping in.
The lead lasted until midway through the second quarter when Oliver Kidd levelled for the hosts with a brilliant drag flick from a penalty corner.
Then, just as the sides were about to go into half-time level a Greg Allen reverse from the top of the circle restored the Cookstown lead before a Jack Haycock stroke in the third quarter doubled their advantage.
However, their two-goal lead was promptly halved with a Harry Archbold strike before Milo Thompson forced the sides to a shootout with a late deflected equaliser.
It was there that the hosts ran out 3-1 winners with goalkeeper James Ritchie proving the hero for his side.
The side will be joined in the Semi-Finals by rivals Banbridge, who progressed with a 5-1 win over Mossley.
Two goals each from Matthew McKee and Johnny McKee saw the visitors cruise to a 4-0 lead by the third quarter.
The hosts were then awarded a penalty stroke, which was calmly converted by Raymond Millar to reduce the deficit to three.
However, a final quarter strike from Louis saw the EYHL side restore their four goal lead as they ran out 5-1 winners and sealed a Semi-Final spot.
In Belfast, Queen’s University relied on a shootout to edge past South Antrim after the sides drew 1-1 in normal time.
A Ben Law penalty stroke, and a goal from Oliver Patterson saw the sides prove inseparable in normal time with a shootout required at The Dub.
With a 3-2 win, it was Queen’s who came out on top, maintaining their unbeaten record this season and setting up a Semi-Final clash with Banbridge.
In the Ulster Premier League, Portadown left it late to beat Belfast Harlequins as they won 2-1 at Edenvilla.
Goalless at half-time, the host took the lead with 42 minutes played when Ryan Bennett scored from a penalty corner to net his X league goal of the season.
Then, with just two minutes remaining in the corner, Matthew Grey drew level for the Belfast side as his reverse strike found the back of the net to send the sides into the final quarter at 1-1.
As the game approached it’s final stages, Luke Bell restored the Portadown lead with a goal from open play and with no Harlequins response, his side ran out 2-1 winners.
